Frederick Alexander Cook the son of F.W Cook and R Cook of Bishops Waltham, Hants. He served in Y / 5 Trench Mortar Battery. Landing France 27th July 1915.

Cook was Killed in Action on the 26th December 1916, which was the same date as the despatch he is mentioned in.
